Home Care Coordinator / Scheduler
 
Aleph Home Care, a department of Jewish Federation of Southern NJ, is looking for an experienced Home Care Coordinator / Scheduler to work in our home care department.

Aleph Home Care serves many south Jersey communities including Camden, Burlington, Gloucester and Cumberland counties.
 
Specific Responsibilities:
  1. Adheres and understands the Aleph policies and procedures
  2. Provide staffing for sick leave, vacation and personal time
  3. Conduct telephone intakes with new and potential clients
  4. Coordinate and schedule aides and clients that match the care plan
  5. Enters scheduling data, client information, communications with client into database
  6. Contacts aides and clients when there are changes in the schedule
  7. Communicate with office staff when clients are discontinuing services and/or pausing services due to hospitalization
  8. Discerns client services required as outlined in their agreement, urgent requests and care plans.
  9. Provide information to the nurses about the clients and coordinate scheduling assessments
  10. Follow up with the clients regarding service delivery within 24-48 hours
  11. Assist with screening and interviewing potential CHHA’s and caregivers
  12. Performs general office duties such as filing, organizing, creating folders for clients/aides
  13. Troubleshoot and problem solve with aides and client needs. Making sure to be a matchmaker between aides and clients
  14. Provide prompt and attentive customer service
  15. Attend regular employee meetings and trainings
  16. On call to coordinate clients and home health aides’ schedules on weekends and weeknights
 
Qualifications:
  1. Associates Degree/GED
  2. Prior scheduling experience in home care is needed
  3. Strong computer skills including, but not limited to, Outlook, MS Excel, MS Word and a Scheduling database
  4. Ability to deal effectively with the high demands of the fast pace needs in homecare
  5. Experience in problem solving
  6. Basic knowledge in medical terminology
  7. Strong interpersonal skills, organizational and communication skills
  8. Must be available to be on call
